Public Health England have requested that all near miss events (RTE) reports use the TSRT9 described in the “development of learning from radiotherapy errors” (2016) in order to reduce variations in interpreting data.

National voluntary reporting of radiotherapy errors and near miss event (RTE) is well established in the UK. Public Health England (PHE) carry out a national trend analysis of radiotherapy incident reports and publish quarterly newsletters and biennial reports to allow the sharing of learning from incidents and enable continual safety improvements.

Changes to the existing DATIX form

The changes for the reporter will be within the event category section and will only be used by the staff in the radiotherapy department when recoding radiotherapy errors and near misses.

All non-radiotherapy incidents will still be recorded using categories already within DATIX. Once TSRT9 has been selected, radiotherapy staff will have access to radiation incident levels and the entire safety barrier taxonomy coding.
